Mishawaka Foundation First Time Applicant

This application is for those interested in receiving a full or partial grant to attend a residential summer camp. This application is specifically for those who have not received a grant from the Mishawaka Foundation previously. You will find a different application for those who have received a campership grant previously available on the Mishawaka Foundation website: mishawakafoundation.org/apply.

The Mishawaka Foundation provides full and partial campership grants to families interested in a traditional, residential camp experience but who could not otherwise afford it. Members of the Mishawaka Foundation are looking for candidates that are in need of a grant but would also be able to benefit from a residential summer camp experience. Any camper attending a traditional residential summer camp will need to be able to effectively interact in a group-based and community living environment. They will be asked to follow instructions, be respectful of others, and behave in a way that is conducive to a healthy group living situation and community.

Candidates for the campership grant should be:

  • Boys or girls between the ages of 8 and 15.
  • In good general physical and emotional health.
  • Able to meet his or her own basic needs, such as eating, getting dressed, maintaining personal hygiene.
  • Able to move independently from place to place.
  • Effectively interact in a group-based, community living environment.
  • Residential camp sessions are typically between 2 and 8 weeks long. Campers attending camp should be comfortable spending time away from home.

Confidentiality and candidate participation in the application process:

Information provided on the application is confidential to the Trustees of the Mishawaka Foundation. The Trustees are authorized by those submitting applications to share confidential information with the Foundation’s professional advisors only on a need-to-know and confidential basis.

Should the applicant be granted a campership and attend a residential summer camp, you authorize the use of photographs taken of the applicant while at camp in future Mishawaka Foundation fundraising materials.

This application intentionally does not require the participation of the candidate, and can be completed without his or her knowledge. Please consider not telling the candidate about the application in case the campership grant is not awarded.

About the application:

There are five steps to the application:

  1. Fill out the some basic information about you and the child you are applying for a campership grant.
  2. Upload supporting documents for your application including a recent photo of the applicant, a recent report card, and the first two pages of your tax return
  3. Sending email link of a questionnaire to the applicant’s sponsor
  4. Sending email link of a questionnaire to a teacher or school administrator of the applicant
  5. Sending email link of a questionnaire to an non-family-member adult familiar with the applicant

Applications are due February 8, 2020. Late or incomplete applications will not be considered. Applicants will be notified by a board member following the annual meeting in March 2020.
